Health Force One offers medical evaluations to discuss your symptoms, review your health history, and develop an individualized care plan. We’ll help determine whether a virtual visit is appropriate or whether you need an in-person evaluation or additional services.
Submitting a request does not confirm an appointment. Our scheduling team will contact you to arrange your visit.
During your evaluation, we’ll discuss the accident, your symptoms, and how they affect your daily activities. Your provider will document the findings and discuss appropriate next steps, which may include treatment, testing, follow-up, or referrals.
Recommendations are based on your medical needs. Testing, prescriptions, and referrals are provided when clinically appropriate.

Contact our team to discuss insurance, self-pay pricing, and whether care under a medical lien agreement may be available for your situation.
A medical lien generally defers payment under a written agreement while an injury claim is pending. It does not mean your care is free or that payment is guaranteed by an attorney or settlement. We’ll explain the charges and payment responsibilities before you agree to this arrangement.
If you would like legal advice about your accident, you may contact an independent attorney. The resources below are optional. You may choose any attorney, and using a listed attorney is not required to receive care from Health Force One.
Legal services are provided independently of Health Force One. Contacting an attorney does not establish an attorney-client relationship; the attorney will explain their consultation and representation process.
Your medical appointment request is separate from these links. We do not automatically forward your medical intake information to the listed attorneys.
You can request medical care regardless of which attorney represents you. With your authorization, our team can coordinate appropriate medical documentation with your legal representative.
Our team can help with appointment requests, payment questions, and the visit process.
